The practice of pain therapy Zurich Paul Hörter, Ich Paul Hörter and employees Michael Schmidt, Bleicherweg 54, 8002 Zurich hereby confirm that us/I comply with the following rules in accordance with the Data Protection Act and Data Protection Ordinance.

The purpose of this privacy policy is for you, as our/my clientin, to inform you about our/my handling of your personal and sensitive data collected in our/my practice.

Only data that is related to the therapeutic treatment or that is relevant to information about any offers will be processed. This is exclusively data that we/I have included with your consent, that you have sent us/me or that can be found in publicly available directories (telephone directory, etc.). This may include general information about you (name, address, telephone number, etc.), information about your health/course of illness, diagnoses you have already made and, if applicable, other data that you have provided to us as a result of the treatment (patient records).

This data is collected, stored and processed exclusively in your patientInternal file, or in the dossier required for administration, in particular in the app required for invoicing in accordance with tariff 590. In these two storage locations, all processing steps carried out on your data, including the persons responsible, can be traced.

We only share your personal data with persons or areas of our practice who need it to fulfill contractual and legal obligations. They are informed about the applicable data protection rules and are required to comply with them.

Your data or parts of it will only be made available to other persons or institutions (insurers, etc.) with your explicit consent. Judicial orders or the enforcement of justified claims by practice are considered exceptions.

Insofar as your data is physically collected and processed, it is stored in a lockable room or closet that is not accessible to third parties. Electronically collected data is stored securely (firewall, password, etc.).

Unless cantonal or other legal regulations state otherwise, your data collected by us/me will be deleted twenty years after your last consultation in our practice.

Written communication between our/my practice and you takes place via e-mail. Either encrypted with a standard encryption program or — with your written consent — unencrypted.

As a current or former client, you can request from us/me a duplicate of all the data we have collected in a standard electronic format at any time. Only physically available data is scanned for this purpose and sent to you as a PDF or in paper form. The data is usually provided to you free of charge and within a maximum of 30 days.

The therapist responsible for you is responsible for all questions relating to the processing of your personal data and exercising your rights,
